TERMS OF REFERENCE

CUSTOMIZED MOLESKINE-TYPE NOTEBOOK (CORPORATE GIVEAWAY)

I. OBJECTIVE
To select the most responsive bidder to supply customized moleskine-type notebook as corporate giveaway.

II. SCOPE OF WORK AND DELIVERABLES

1. Supply and deliver One Thousand (1,000) pieces of customized moleskine-type notebook;
2. Submit sample customized moleskine-type notebook and agree on final product look before actual production;
3. Pack customized moleskine-type notebook singly in HGC paper bag/ wrapper with greeting card for ready distribution. (Design of HGC paper bag/ wrapper to be provided by HGC); and
4. Replace items found to be defective because of supplier’s fault.

1. Emerald green hard cover;
2. Approx. Size: 13 x 21 cm;
3. Rounded corners;
4. Elastic closure;
5. With ribbon bookmark;
6. Ivory colored ruled pages;
7. Number of pages: Not less than 100 pages;
8. Smyth sewn perfect bind;
9. With printed 2017 Calendar, HGC mission, vision, programs & contact details before 1st page;
10. With HGC name printed in the middle front cover & HGC logo preferably embossed or printed on the middle low back cover

IV. APPROVED BUDGET FOR THE CONTRACT (ABC)
Three Hundred Thousand Pesos (P 300,000.00) Inclusive of VAT (packaging included) chargeable against 2016 Corporate Operating Budget

V. DELIVERY PERIOD
30 days upon approval of final sample

VI. OTHER REQUIREMENTS

1. Sample moleskine-type notebook that will be used for evaluation of proposals; and
2. Certificate of PhilGEPS registration.

The bidder with the lowest proposal and most responsive to HGC’s requirements shall be awarded the contract.
